Step 1 - Request an Appointment
If you have not yet scheduled an appointment, you may do so by submitting the booking form or by calling us.
Step 2 - Download and Print New Patient Registration
After you schedule your appointment, we encourage you to download and print our new patient forms, so you can fill them out at your convenience. You can fax or mail them to us, or bring them with you to your first appointment.
Step 3 - Get Directions
View driving directions on our contact page, you may call our office at to get more detailed directions. That's it! We will see you at your first appointment!
Dental Insurance
Accepted Insurances
We accept most major insurances, but please contact our dental office for information. We accept all PPO dental plans. Our office does not currently work with state insurance plans. Please ask our office manager during your visit if you have any questions about the insurances we accept. We participate with:
- Aetna PPO
- Ameritas PPO
- Cigna PPO
- Delta Dental PPO
- MetLife PPO
We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.

We accept major credit cards and offer CareCredit or payment arrangements. Please ask our front desk about our affordable dental care. We are here to help! Request an appointment now by clicking on the Contact Us button above or calling our Fall Hill location or Falcon Drive location.
Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. For your convenience, we accept:

If special arrangements are needed, please talk to our office manager prior to receiving service.
Patient Forms
Going to the dentist can be an overwhelming experience, especially on your first dental visit. Let us put your concerns and anxieties to rest. Our goal at Heresi Dental Care is to make every patient experience as comfortable as possible.
To make your visit as pleasant and brief as possible, there are a few things you should bring with you. Please bring any dental X-rays from the last six months to your first appointment.
With just a few simple forms, we’ll ask for relevant medical history, any medications you might be taking, your dental history, and any dental insurance coverage you may have.
